This article provides instructions on how to collect the required information for completing the AMD Product Warranty Claim Form.
Providing all the requested information will help us quickly evaluate your warranty claim. Please note that the information requested on your claim may vary depending on your product and issue.
AMD PIB Processor Part Number (OPN) and Serial Number (S/N)
The Processor’s Part Number (OPN) and Serial Number can be found on the product packaging and on the surface of the processor. The image below shows where this information can be found.
In the example above, you would enter the following:
OPN: YD170XBCAEWOF
Serial Number: 9G ########### *
* Each processor has a unique serial number; the hash symbols used in this example are for illustrative purposes only.
Important note:
- For PIB processor OPNs beginning with 100, simply enter the last four digits in the warranty form to quickly filter and select the part number matching your processor. For example, an AMD Ryzen™ 7 5800X3D PIB processor with OPN 100-100000651WOF, enter 0651 into the Part Number (OPN) field for matching results.
- For PIB processor OPNs beginning with alphabetic characters, enter the first six characters in the warranty to quickly filter and select the part number matching your processor. For example, an AMD Ryzen™ 7 1700 PIB processor with OPN YD1700BBAEBOX, enter YD1700 into the Part Number (OPN) field for matching results.
In the example shown above, the make and model of the motherboard is: ASRock X370 Taichi
The motherboard make and model should also be printed on the retail box.
In the example shown above, the make and model of the motherboard is: ASUS Prime X570-Pro
BIOS Version
The motherboard BIOS version is available in the system BIOS, which is accessed by pressing the appropriate hotkey (prompted on the screen), during system bootup.
The example BIOS menu shown below indicates that the BIOS version is 2602 for an ASUS X570 motherboard.
If you are unable to boot the system, the original BIOS version may also be found in the motherboard user manual or on a label attached to the motherboard. As shown in the example below the BIOS version is 0411.
In the example above example, you would enter the following:
Part Number: 102C9540100
Serial Number: 181### ###### *
* Each serial number is unique; however, the actual serial number of the processor is not displayed in the image above.
Please also include authentic photos of your made-by-AMD Radeon™ Series Graphics that clearly shows the model number, part number, and serial number with your warranty claim, as shown in the examples below:
AMD Desktop Kit Part Number (P/N) and Serial Number (S/N)
For AMD Desktop Kit purchased as a sealed, retail-packaged product the part number and serial number should be available on the sticker attached to the retail box.
In the example below, the AMD Desktop Kit product part number (OPN) is 100-190000005BOX and serial number is 26##########. Each Desktop Kit has a unique serial number; the hash symbols used in this example are for illustrative purposes only.
AMD Processor Cooling Solution Part Number (P/N)
If your AMD processor cooler was bundled with an AMD PIB processor, use the Part Number (OPN) of the PIB processor when making a warranty claim. To locate the PIB processor Part Number (OPN), follow the instructions mentioned in the AMD PIB Processor Part Number (OPN) and Serial Number (S/N) section.
If your AMD processor cooler was purchased as a sealed, retail-packaged product such as the AMD Wraith Prism, the part number and serial number should be on a label attached to the retail box. In the example below, the part number for the AMD Wraith Prism Retail Box cooler is 199-999888.
